High Risk of Diabetes

Prediabetes

Individuals with blood glucose or HbA1c levels above normal but below the diagnostic threshold for diabetes. Often has no symptoms.

  • Raised HbA1c or fasting glucose
  • Often asymptomatic
  • Usually not taking diabetes medication
  • Increased risk of progression to type 2 diabetes

Goal: Reverse abnormal glucose levels or delay progression

Group 3

Diagnosed Type 2 Diabetes

Individuals who have received a formal diagnosis of type 2 diabetes and require ongoing management.

Your fee covers comprehensive professional support over 12 months, including:

  • Initial 1-1 Consultation with an Expert & Full Health & Risk Assessment with a Back to Basics of Diabetes Education and Understanding Diabetes Information Pack at the start of the Programme
  • A Full screen for specific diabetes risk markers like Insulin, Fasting Glucose, C-Peptide, ApoB, hs-CRP, Urine ACR
  • A Bespoke Personalised prevention and remission (month by month) plan with monthly support tailored for you
  • Monthly consultations with our Clinical Team
  • 3-monthly blood tests to include HbA1c with in-clinic consultations with an expert to monitor progress
  • Blood Tests will include Insulin Resistance Tests and Fasting Glucose
  • Free monthly supply of Vitamin D, B12 and B9 if required
  • Nutrition and lifestyle guidance from a Nutritionist/Dietician
  • Cardiovascular risk assessment including BP checks and a 1-lead ECG
  • Full Medication review (where appropriate)
  • Ongoing monitoring and progress tracking
  • Two GP consultations at no additional cost within 12 months
  • 10% off any Weight Management Programmes using GLP Injections if Weight Management is Recommended as part of the programme if you are high risk and your BMI is >27
  • Final annual review and long-term plan
